Was she taking daily asperin prior to starting warfarin? My mother had been taking this for many years prior to being put on LMWH after a PE. After a few months she started getting crippling cramps.. One of the many doctor's opinions was that the asperin was doing all sorts of anti inflamitory effects while she was taking it. After a few months off of it, she essentially went into withdrawal from the asperin. She did start to take Quinine which helped to ease the severity of the spasms but is still getting them. So it is not so much the warfarin causing the problem, but the withdrawal from asperin.
