i did too and my doctor refuses to remove after 2 weeks because of the cost, but i paid my co-pay. do you know what the waiver is about?
Not only do I agree with you about "DR's possibly getting a kickback" for promoting these devices, (including uterine ablation),
but I think that they "get punished" for every removal that they do that is not part of the "must be removed within 5 years" cycle.
I'm very curious to see if there is a "log" that they have to submit and to who it goes to. THAT information should be public.
I had to fight tooth and nail for them to remove mine, starting with the receptionist who battled me over insurance coverage ("oh, they don't cover removals") to the DR that inserted it. (they refused after my first demand and could not refuse when I didn't stop calling them.

I did sign a waiver however I have no idea what I signed nor did I keep a copy. I hope my OB is not selling her soul to the pharmacy companies at the expense of her patients.
