I am almost 41, and have had migraine headaches most of my life which I could manage with 800mg of Naproxen Sodium, or a Norgesic Forte, and when Imitrex came out, I could just administer that. Six months ago, my headaches got so frequent that I had to start taking my Relpax (similar to Imitrex) and/or Fioricet more than 2X per week. According to my neurologist, this is a no no, as both of those meds can couse rebound headaches. He started me on Topamax, 50mg per day. It did nothing. He bumped it to a 100, nothing. Now I'm a 200 mgs, I am still have a migraine in left temple as I write this. I have the tingling, the daytime sleepiness, but my mind is fine, and my appetite is just swell (regretfully). What the hell am I going to do now? Ask him to bump me up to 400 mg? Now I have these headaches every "F"ing day, and I can't take anything. I just have to suffer. Topamax is NOT working!!!!!

I am so, so sorry about your pain. I read in a book about migraines that people who suffer from cluster-type headaches can get help from medical-grade oxygen. I have also heard of migraine sufferers producing too much spinal fluid and that this puts pressure on the brain, causing pain. A colleague of my husband has treatment for this, in which some of her spinal fluid is drained. I have also heard of migraineurs having PFO (patent foramen ovale), a hole between the left and right atria in the heart which doesn't close. Although this isn't an uncommon condition, there is some speculation that this can cause migraines. Apparently, having this hole repaired can cause marked improvement. Other types of treatment that might work are massage and accupuncture. Terry Roberts has written a good book on helping yourself with migraines; it includes lots of different medications.
I am 42 and have been getting migraines since I was 8. I am on 100 mg. of Topamax and take Maxalt as an abortive. I haven't had much success with this treatment so far. The Maxalt only takes away what I consider to be my "baby" headaches, and the Topamax isn't really preventing the headaches, plus I have chronic daily upper and lower GI symptoms, and difficulty spelling and doing mental figuring. My migraines are severe and frequent enough that I sometimes worry I will have to be on disability.
It sounds like you are suffering similarly. I don't know if I've given you any other ideas at all, but I wish you the best. I truely hope you find relief. Try hard not to give up, and try to keep searching for therapies that work for you.
I'm sorry your head hurts today, I hope it doesn't last much longer.
