Medic Alert ID for ADR to Levaquin?

I have recently experienced tendontitis and earache as an ADR to Levaquin. My doctor tells me I should not take Quinolones again as my ADRs may be even worse the next time. I'm thinking about getting a Medic Alert ID to be sure Quinolones are not administered if there were to be an emergency situation. Has anyone out there gone this route? If so, what does your Medic Alert ID read?

Well, I had no response, so I decided for myself. In case anyone finds this posting in their own search for answers: I did get a medic alert ID. My doctor tells me my reaction could be classified as a "serious adverse drug reaction" so my medic alert ID reads "serious ADR to QUINOLONE" on the backside.
