problem with samples from pcp

I rec'd several samples of Aciphex from my PCP. Each bottle contains 5 pills. 20 mg. Problem is, there is no dosage listed. On the front of the bottle, it states, ...dosage and use, see accompanying prescribling information. The paper insert in the box is for a $30 refund. No other product info. What is the recommended dosage? Thanks-Greg
