Clinical trial of montelukast in the Netherlands.
I noticed two things:
1. The researcher states that sides effects are 10%.
2. The researcher will not allow patients to also take drugs which are metabolized by CYP2C8 because montelukast inhibits that as proved by in vitro (test tube) studies. American studies in vitro said yes montelukast is an inhibitor but in vivo (in people) that it didn't happen. I was always confused by that and would still like to know more

Concernedcitizen - I am not an expert in this area. I am truly sorry that somebody out there who actually is an expert hasn't stepped up to the plate to look into these problems. I didn't put that molecule (montelukast) together so I don't know how it comes apart. I don't have any answers. The human body is just chemistry and we don't understand everything.
There IS a reason that montelukast causes adverse side effects for SOME people. In my opinion, the reasons will be in two categories 1) genetic factors related to the receptor and 2) metabolic factors such hypoglycemia or enzyme deficiencies. In the first case, montelukast would not bind to the receptor. The receptor is a gene and there are many sub-types. In the second case, blood PH could cause other chemical reactions or enzyme deficiencies would cause montelukast to act differently than the expected pattern of the break down and elimination of the molecule.
The drug categories that included genetics as part of the efficacy profile were the wave of the future. It is completely and totally inexcusable that Merck allowed their marketing department to take a very complicated concept, that required that doctors know how the drug works and what the side effects are, and "dumb it down" into cherry flavoring, chewable tablet and once a day dose. There is no such thing as a drug without side effects. How can doctors monitor a patient without proper communication from the people who invented the drug?
The other BS that goes on is drug companies that claim that they don't know exactly how a drug works. Some drugs took up to twenty years to create. So they don't know???? These companies are highly competitive. If they don't want to say how a drug works for fear of competitive, well too bad. The policy should be that the mechanisms are stated clearly for all doctors to read. Clinical diagnosis of a patient always requires knowing what the possibilities are and then eliminating them one by one.
You, Kate, are trying to do something for others that is probably next to impossible. That something is to get the government to listen to the people rather than the lobbyists. I believe that eventually you will have many on your side. The information that I found is available to everyone. I wish that I could get my hands on some of the stuff that is not available.
I know that this site has a policy against posting links to other sites. I thank them for their generosity and their concern for those with side effects. I cannot always find a way to refer to information that doesn't violate their rules so I just let them delete it if they have to. We should just all understand that they have to follow their policies.
Maybe they will allow a direct link to the government clinical trial site instead. I didn't notice that I wasn't looking at a site that they would NOT allow.

Flindy - I understand this is chewing at your heart and guts,but your postings have been invaluable,,i am taking matt on wednesday for our first face to face with the pulomi doc, i have not seen him since the news broke but have spoken to him a couple of times on the phone,I was not ready to see him and told him that as i can not in my head phathom how he did not know.I am going to ask lots of questions,questions i am sure as only a mother he will not expect me to ask,but i have you on my side,bring up the questions that need to be asked.Please take the time to take care of yourself,you have made a difference
